چکیده

It is generally acknowledged that a reduction in the sensitivity to penicillin of several gonococcal strains is leading to an increase in therapeutic failures. Since 1963 we have tentatively used ampicillin (Doctacillin 8) to treat gonorrhoea infections in patients in the Copenhagen prisons. The acid stability of ampicillin, the satisfactory absorption of the drug from the gastrointestinal canal, the fact that its free concentration in plasma is higher than that of penicillin G (owing to its reduced binding to plasma proteins), and its intensified bactericidal action on gonococcal strains with reduced sensitivity to penicillin G (Odegaard, 1962) provide the theoretical basis for the application of the drug. In the treatment of imprisoned patients the finding of positive cultures after treatment always points to therapeutic failure, whereas among out-patients it may be difficult to decide between treatment failure and re-infection (Willcox, 1964). Accordingly, the present series, in which all the patients were isolated in hospital or in prison awaiting trial, provided ideal data to assess the efficacy of ampicillin therapy.

Treatment of gonorrhoea by one oral dose of ampicillin and probenecid combined.

It has been shown (Odegaard, 1962) by sensitivity testing in vitro that the effect of ampicillin on most gonococcal strains is somewhat weaker than that of penicillin G, but the effect on strains less sensitive is better with ampicillin than with penicillin G. Reyn and Bentzon (1968), in support of this, found that the 50 per cent. inhibitory concentrations for the naturally-occurring more sens...

Ampicillin serum levels and treatment results in gonorrhoea.

Recent studies have proved that the treatment of uncomplicated gonorrhoea with oral ampicillin, administered either in a single dose of 2 g. plus 1 g. probenecid or 2 g. divided into two doses at an interval of 5 hrs, is an effective alternative to the conventional treatment with a single intramuscular injection of 2-2 m.u. penicillin G (Eriksson, 1970a,b, 1971a). To find out if there is any co...

Resistance in Escherichia coli after single dose ampicillin to treat gonorrhoea.

Patients with uncomplicated gonorrhoea were treated with a single dose of either ampicillin 3 g orally or procaine penicillin 2.4 MIU by injection, both with probenecid 1 g orally. The proportion of faecal Escherichia coli resistant to ampicillin before and a week after treatment was assessed. Of 55 patients treated with ampicillin who initially possessed sensitive flora, 25 (45.5%) became colo...

Diagnostic treatment and reporting criteria for gonorrhoea in sexually transmitted disease clinics in England and Wales. 2: treatment and reporting criteria.

The current methods of treatment and reporting are described. The most common form of treatment for gonorrhoea in men and women was a single dose of oral ampicillin, usually 2 g. In proportionately more of the clinics treating women, treatment was given over several days. The wide use of ampicillin represents a marked change in treatment practice during the last decade. Procaine penicillin was ...

Treatment of gonorrhoea and susceptibility to antimicrobials of PPNG and non-PPNG strains in Jamaica.

Of 1400 patients in Jamaica screened for uncomplicated gonorrhoea, 54% (753 patients) were culture positive. Of the 459 patients who complied with the terms of the study, 97% (211/218) of those treated with aqueous procaine penicillin G were cured compared with 94% (227/241) of those treated with ampicillin. Penicillinase producing Neisseria gonorrhoeae (PPNG) strains were identified for the fi...
